The Middle Spotted Woodpecker excavates its nest hole in rotten, coarse deciduous wood, often in old oaks. The female usually lays 4–7 eggs. The species is strongly dependent on a supply of dead and dying wood in sunlit, open deciduous forests, which makes it sensitive to modern forestry. There is no regular breeding in Sweden today.
