The Eurasian Three-toed Woodpecker nests in a nest hole excavated in a dead pine or a dead spruce, usually two to five metres up the trunk. The female normally lays 3–5 white eggs, and both parents share the incubation, which lasts about two weeks. The young are fed by both parents and fledge after about three weeks in the hole. The pair breeds with one brood per year.
